Accepted presenters and performers agree to appear at the New York Flute Fair at their own expense (which will include the flute fair registration fee).

Notes regarding equipment and bios:

1. Equipment requests submitted with this form will be accommodated where possible. Please be sure to include the number of music stands you will be needing. Last-minute changes are not allowed.

2. Do not use a link to your website as a substitute for a program book bio. However, you are encouraged to Include your website url at the end of your bio. Please use a "just the facts" style and avoid using quotes from reviews. Sample bios can be found in program books from recent flute fairs (archived at www.nyfluteclub.org/about/history-and-archives/flute-fair-archives).

3. The requested headshot photo is for use in our publicity materials. Please make sure that the filename of the submitted photo starts with your last name, followed by a comma and your first name.
